COVID-19 Protocol
If your child is exhibiting any Covid-19 related symptoms and requires a doctor's visit, please print the Covid-19 Clearance Form which must be completed by your doctor (see list of symptoms on the form).
Before your child attends school, the COVID Clearance Form below must be completed by a healthcare professional. This form MUST be received via fax or email before your child is allowed to enter the school building.
Thank you for your adherence to this policy as we take every measure to ensure the safety of our students and staff.
