The district’s Health Benefits Office handles enrollments in:

- Health Insurance, COBRA health
- Dental, COBRA dental
- Life Insurance
- Long-Term Disability
- Flex Spending

Harborfields CSD offers NYSHIP The Empire Plan through United Health Care to its employees. (UHC App or visit https://member.uhc.com/myuhc)
Eligible employees are offered health insurance on their first day of employment. The date coverage begins depending upon when the employee is hired. Coverage always starts on the first day of the month.
After the initial hiring process, the only time an employee can make changes to their policy is when a qualifying event happens. The benefits office needs to be notified within 30 days of any changes throughout the year. See Qualifying Event for more details.
Open Enrollment in November is the only time of the year where you can make voluntary changes.
Payments for healthcare will be made during the school year over 12 (21 for TAs) payroll deductions. This will allow for the collection of premiums for July and August.

Harborfields offers two types of plans under Equitable’s Dental Insurance. Group #020115.
Open enrollment is in June with July 1 effective date. Only time when you can make voluntary changes.
DHMO Plan
- Service is through the Solstice network
- Benefits are available for In-Network providers only
- A Primary Care Dentist (PCD) will be required at enrollment and unless a specialist (e.g., oral surgeon) is required, all services have to be through her chosen PCD.
- Members have preset copays
PPO Plan
- Service is through several networks that Equitable leases
- Benefits are available for both In-Network and Out-of-Network providers, though members pay less when they use In-Network providers
- Members can visit any dentist
- Cost to members is determined by the plans deductibles and coinsurance.

Life insurance is provided by First Reliance Standard Life Insurance Company through J.J.Stanis and the policy amount is indicated in the individual contracts and policies.
We offer Non-Contributory and Contributory life insurance.
Non-Contributory is fully paid by the district and Contributory is fully paid by the employee.
Life insurance is offered upon hiring and can only be changed during pen enrollment period or when a qualifying event occurs.

Employees are eligible to enroll in AFLAC voluntary benefits immediately upon hire through payroll deduction.Enrollment Process:

If you do not enroll at the time of hire, the next opportunity to enroll will be during Open Enrollment, which occurs annually in September. Open Enrollment Memo will be sent out districtwide.- Coverage elected during Open Enrollment will become effective October 1.
Important Notes:
- Enrollment cannot be completed online or through HR — you must go through James Maloney.
- Aflac offers a variety of supplemental insurance options, including accident, critical illness, short-term disability, and more.

Long-Term Disability is provided by Mutual of Omaha to Harborfields’ employees upon eligibility. It is 100% district provided upon hiring.

Turning Age 65: What You Need to Know About NYSHIP & Medicare
When you or your dependent turns age 65 and becomes Medicare-eligible, what you need to do depends on whether the NYSHIP policy holder is actively employed or retired.
Medicare Basics
- Medicare Part A (Hospital Insurance) Typically no cost.
- Medicare Part B (Medical Insurance) Monthly premium paid directly to Medicare (CMS).
- Medicare Part D (Prescription Drug Coverage) Monthly premium paid directly to Medicare.
Important: If you are covered under the District’s NYSHIP plan, do not enroll in Medicare Part D. Prescription coverage is already included. Enrollment in Part D will result in termination of your NYSHIP coverage.
Active Employees (and Dependents) Turning 65
Understanding Medicare as an Active Employee
You will receive a letter from NYSHIP outlining your options.
As an active employee, your NYSHIP coverage remains your primary insurance.
What This Means:
- You are not required to enroll in Medicare.
- You will not be reimbursed for Medicare Part B premiums if you choose to enroll.
Optional:
Many active employees enroll in Medicare Part A only, since there is typically no cost. This can make it easier to add Part B later when you retire. Do not enroll in Medicare Part D.
Active Employees Planning to Retire at Age 65
If you are planning to retire, you must have Medicare Parts A and B effective on the first day of the month following your retirement date. Please review the Retiree section below to ensure proper enrollment and avoid coverage issues.
Retirees (and Dependents) Turning 65
You will receive a letter from NYSHIP explaining your responsibilities. Once retired and Medicare-eligible:
- Medicare becomes your primary insurance
- NYSHIP becomes your secondary insurance
Your NYSHIP carrier will assume you are enrolled in Medicare and will only pay after Medicare has paid its portion.
Required Enrollment:
- Medicare Part A
- Medicare Part B
Failure to enroll may result in reduced benefits or unpaid claims.
Medicare Part B Reimbursement (Retirees Only)
Harborfields CSD reimburses the cost of Medicare Part B premiums.
To Begin Reimbursement:
- Submit a copy of your Medicare card.
- Complete and return the District’s reimbursement form.
Reimbursement includes:
- The standard Part B premium
- Any income-related monthly adjustment (IRMAA), if applicable
Prescription Drug Coverage Reminder
Your prescription coverage continues through NYSHIP. Do not enroll in Medicare Part D. Part D premiums are not reimbursable, and enrollment will terminate your NYSHIP coverage because these plans cannot be combined.
